Missing appointment details

You should contact NHS Scotland if you think you should have received an invitation to your coronavirus (COVID-19) vaccination appointment by now, or if you have lost your invitation letter.

You can do this by phone or by completing an online form.

NHS Scotland are aiming for everyone in JCVI groups 1-9 to have their first dose of the coronavirus vaccine by mid-April 2021.

If you are aged 50 to 64 years, you should expect to receive your first dose coronavirus vaccination invitation letter soon.

NHS Highland You should complete an online contact form if you are registered with one of the following GP practices:

  • Glen Mor Medical Practice
  • Tweeddale Medical Practice
If you're registered with any other GP practice, you should phone them to enquire about your missing invitation.

NHS Tayside If you're 65 and over, phone your GP practice to enquire about your missing invitation.

Before you complete this form, check that you:

  • fit into one of the priority groups currently being invited for vaccination (groups 1 –9)
  • live in a health board area that recommends contacting them via this form

You should use the form to provide contact details and an explanation of why you think you should have received an invitation.

This will help NHS Scotland to confirm your eligibility and get back to you with a letter or information on how to arrange your vaccination appointment.

Once you have submitted the form you will see a receipt. You will receive an email with further details of what will happen next after the team has triaged your request.
