Care Management - Aberdeen City Health and Social Care Partnership
Care management works with any adults over the age of 16 with a physical disability, neurological condition or complex needs living within the community or in care environments. It consists of 2 service managers, 6 seniors, 4 senior practitioners, 35 care managers and 16 care co-coordinators. It works in 4 teams under the 4 localities of the adult health and social care partnership as well as having the care management response team, development team and adult support and protection unit.
Care managers can be qualified social workers, nurses and occupational therapists. Staff are based within Marischal College, throughout GP practices within Aberdeen City and within the horizons rehabilitation centre.
