Adult Community Mental Health Team

													

Specialist team of nurses and healthcare support workers who have in-depth training, experience and understanding of mental health difficulties.

They can:

• Assess, treat and monitor mental ill health

• Offer a variety of therapeutic interventions

to help people overcome their difficulties

• Promote good health

• Refer for hospital admission if required

• Access to the service requires a referral mainly from:

a Primary Care Mental Health Nurse

a GP

an Advanced Nurse Practitioner

or via the Accident and Emergency Department at the Galloway Community Hospital
